Alternatively, on your next visit you can speak to one of the friendly team at the practice who will be able to tell you whether we are taking new patients onto our Membership Plan (there are a limited number of spaces available).

If we have spaces available within our Membership Plan, we will provide you with an agreement form which will require your signature and bank account details for the Direct Debit Mandate.

Direct debits are collected from your account on the 8th of every month. Any patient enrolling before the 27th day of each month will benefit from cover from the 8th of the following month.
